The general principle of the transplant is to move hair follicles (follicles) from the donor areas of the patient to the areas to be replenished with hair. The follicles are transplanted with grafts – groups containing 1-6 bulbs. To achieve a natural hairline and the entire area to be transplanted, grafts of different sizes are formed. Mini grafts contain up to 6 bulbs, and micrografts – no more than 3. It is micrografts that are used to create a hairline, which makes it smooth and natural.
The donor area is usually the back of the head and the upper part of the neck. Hair can be transplanted to another area of the scalp, to the chin, brow ridges. The operation takes place under local anesthesia and does not require hospitalization. Depending on the technique and the size of the area to be filled, it takes 2-8 hours. At the end of the transplant, the surgical field is closed for the healing period (it is different for different methods) with a special bandage that prevents infection and facilitates engraftment of grafts.
There are several hair transplant techniques:
strip method (from the English “strip” – strip) is called so because of the shape of the grafts, which are strips of skin with hair follicles. They are excised with a scalpel, then divided into grafts, and already in this form are transplanted into the planned area. This also requires small incisions in which the grafts are placed.
This is a classic, long-standing method. It is still used today and is the most inexpensive. However, it has some disadvantages. First, at the site of the excised stripes of skin, scars are subsequently formed, requiring hairstyles with long hair to hide them. Secondly, the healing process takes several weeks and is accompanied by discomfort.
Thirdly, in the postoperative period, the inflammatory process is not excluded. Fourthly, in this way, at best 40 hairs can be transplanted into 1 cm 2, while the natural density is 80-120 pieces. Fifth, it is impossible to control the depth of transplantation and the direction of hair growth in the future.
Therefore, the hairline is not too thick and even; the FUE method (an abbreviation of the English expression “follicular unit extraction”, which literally means “extraction of follicular groups”) consists of the extraction of grafts from the donor zone, ready for transplantation. It is less traumatic and more effective. The smallest fragments of skin with several follicles are removed in a semi-automated way using a special instrument with a rotating tip. In the donor area, the smallest defects, only a few millimeters in size, remain, so they heal almost without scars and much faster than in the classical version. Implantation is carried out using a special Pen Fue device, which allows transplanting up to 60 hairs per 1 cm 2;
HFE method (an abbreviation of the English phrase “hand follicular extraction”, meaning “extraction of follicles by hand”). This is the most advanced technique without the use of a scalpel or other cutting tools. Hair follicles are removed manually with a special device with a tip resembling a hollow needle. The grafts obtained by this method consist of 1-3 follicles, the wounds in the donor area do not exceed 1 mm in size and heal without any scars in a few days, without causing unpleasant sensations to the patient. The risk of infectious complications using this method is minimal. Graft transplantation is carried out during the puncture with a special implanter, and the doctor can control the depth and direction of the transplant, so the hairline in the future will have a smooth and neat appearance. This is the only technique2.
